Online Informed Consent Form for Adult Participants
Dear Participant,
We are a group of students from the University of Hong Kong, taking the course CCCH9051 Digitizing Cultural Heritage in Greater China. As part of the group project assignment of the course, we are creating a digital gallery on Long-Established Hong Kong Eateries and are collecting our target users’needs. To collect your needs on this digital gallery, we will invite you to participate in a 5-minute questionnaire survey. The responses will be recorded and subsequently delivered for further data analysis.
This procedure involves no greater than minimal physical, psychological or privacy risks. All data collected will be used for academic research purposes and can only be accessed by our student project group and possibly the teachers of the course CCCH9051. Personal identifiers will only be used for documentation purpose and will be deleted by the end of the course. All data will be kept confidential in a password-protected computer, including the course teacher’s computer located in his/her office. Strictly anonymized data will be retained for potential use of the researchers and the wider research community.
Participation in this study is entirely voluntary. You have the right to review and erase the data recordings. You will be free to withdraw from the procedure at any time without any negative consequences.
